American Canyon House Calls, P.C. is a medical group of dedicated, experienced highly trained home care Physicans and Nurse Practitioners, who specialize in the medical care of individuals with chronic medical conditions in the home and long term care settings (e.g. assisted living facility, board & care, and skilled nursing facilities).
Care Coordination.
Our providers will communicate and coordinate your medical care with your primary care physician or specialist, along with other health care professionals (home health nurse, nurse case manager, pharmacist, physical therapist, caregiver, social worker, hospital discharge planner, etc.). This vital care coordination will also occur during transitions periods when you are transferring from hospital to home or vice versa.

Our providers have special training in treating medical conditions affecting the elderly and have credentialing and/or have board certification by their respective professions in geriatric medicine.
Avoiding the hospital
Avoiding the hospital
by closely monitoring your chronic medical conditions in your home, our providers are often able to manage the acute exacerbations of these chronic medical conditions with urgent home visits. Helping to provide early diagnosis and along with effective and safe treatment modalities in the home, these exacerbations will not require treatment in the acute care setting (e.g. emergency room and hospitals) |
